Newly seeded lawn starting to die?

Hi all,

I had a few patches on my lawn, so laid some seed around the end of August beginning of September.

The seeds grew and looked really good and healthy. However, in the last week or so the newly seeded grass is beginning to yellow.

Any advice on what to do?
